ADVANCE DIRECTIVE

An advance directive speaks for you if you are unable to speak and helps to assure that your religious and personal beliefs will be respected. It is a useful document for an adult of any age to plan for future health care needs.


The North Queens Surgical Center accepts patients for services who are otherwise in good health.


  1. Patients are evaluated by physicians and nurses prior to the surgical experience or procedure.

  2. Although we do not expect any problems or complications in your scheduled procedure(s), an unforeseen complication could occur.

  3. At the time of the event, we would immediately institute emergency medical care, including but not limited to, life support and transfer you to the closest acute care hospital for follow-up care.

  4. Please bring a copy of your Advance Directive with you on the day of surgery
